• Review physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required. Evaluate, care for and direct treatment for patients. • Plan, prepare and perform individually designed treatment programs to maintain, improve and/or restore physical functioning, alleviate pain and/or prevent physical dysfunction in patients. • Identify and document goals, anticipated progress and plans for re-evaluation. • Evaluate effectiveness of treatment at various stages and adjusts treatment to achieve maximum benefit. • Provide patients with a meaningful rehab experience utilizing excellent clinical skills and customer service. Job Requirements

Qualifications:
• Bachelor’s degree (B.A.) from a college or university accredited by the Commission on Accreditation of Physical Therapy Education (CAPTE). • Position requires a minimum of two years job-related experience or equivalent combination of education and work experience. Prior clinical experience a must. • Strong understanding of state, federal and local regulations pertaining to outpatient rehabilitation. • Understanding of clinical documentation requirements utilizing electronic health records for Medicare and other payors in accordance with guidelines established by Medicare, Medicaid, other payors and the APTA. • Strong communication skills for accurate, concise, and organized verbal and written interactions with patients and co-workers.
